Printable Truck Accident Checklist

Call 911 immediately to report the accident and request emergency medical services.

Check yourself and others for injuries. Do not move anyone who is seriously injured unless they are in immediate danger.

If possible, move your vehicle to a safe location off the main road and turn on your hazard lights.

Do NOT admit fault, apologize, or make excuses to anyone.

Use your phone to take photos and videos of everything: all vehicle damage (from all angles), license plates, skid marks, road debris, traffic signs, weather conditions, and any visible injuries.

Politely exchange information: Driver's name, address, phone number; Insurance company and policy number; Trucking company name, address, phone number, and USDOT number (visible on the truck cab).

Get contact information from any witnesses. Ask what they saw and if they are willing to speak with the police.

Seek a full medical evaluation from a doctor or hospital, even if you feel fine. Some serious injuries have delayed symptoms.

Follow all medical advice, attend all follow-up appointments, and keep all medical records and receipts.

Report the accident to your own insurance company. Provide only the facts, do not speculate on fault.

Do NOT give a recorded statement to the other party's insurance adjuster without first consulting a lawyer.

Start a journal to document your physical symptoms, pain levels, emotional state, and any ways the injuries affect your daily life.

Keep a file of all accident-related documents: the police report, medical bills, receipts for expenses, and correspondence from insurers.
